My Support and Care began with a handful of support workers who kept meeting the same participants — people whose plans looked fine on paper but whose lives were being organised around rosters, not routines. Staff changed weekly. Nobody knew their name.
So we built the opposite kind of provider: small matched teams, team leaders who are accountable, careful planning where it matters, and honest communication with families even when the news is hard. We specialise in the supports that are genuinely difficult — complex and high-intensity care, forensic pathways, hospital discharge, behaviours of concern — because that is where good practice makes the biggest difference to someone's day.
If a support plan does not survive a bad Tuesday, it is not a plan. It is paperwork.
